The facility had a “perfect” (with 0 deficiencies) survey from the Portage County Health Department and had a “near perfect” survey from the Ohio Department of Health with a single deficiency related to a minor maintenance issue that was corrected immediately during the survey.

Following a four-day survey that ended on May 7th, 2015, Anchor Home Health of Greater Columbus was awarded a “deficiency- free” survey by the Ohio Department of Health.
